L’ antigene prostatico specifico è una proteina organo-specifica prodotta dalle cellule della prostata e ha la funzione di fluidificare il liquido seminale. Il dosaggio del PSA rappresenta uno step fondamentale nella diagnosi precoce del tumore della prostata, ma da solo non è sufficiente a diagnosticarlo. Il PSA, infatti, non è un marcatore tumore specifico: i suoi livelli sono influenzati dall’età del paziente, dalle dimensioni della prostata e possono aumentare anche in presenza di alterazioni della ghiandola come prostatite (infiammazione della prostata) e ipertrofia prostatica benigna. Il rapporto tra il PSA totale e la quota libera di PSA aumenta la specificità dell’esame. Il livello di PSA libero, già normalmente ridotto, tende a essere più bassa in presenza di carcinoma prostatico.
